I give permission for my child/children to participate in all vacation Bible school activities. I indemnify and save Independent Baptist Church, its affiliates, volunteers, and agents harmless from the against any claims, demands, causes of action, liability, medical payments, costs, and attorneys' fees resulting or arising out of the participation by my child in the activities of vacation Bible school.
